1.Program purpose:<\/strong><\/p>\n

a)Support pediatric patients in poor, near-poor and disadvantaged households, who are suffering from cardiovascular diseases to be examined, operated and treated at Children’s Hospital 1 and Children’s Hospital 2.b)Contribute to bring the strong heart for pediatric patients across the country.<\/p>\n

 <\/p>\n

2.Subject and pathology group:<\/strong>
\nPediatric heart surgery, including cardiovascular diseases such as: ventricular septal defect, atrial septal defect, cor-triatriatum dexter, mitral valve regurgitation, tetralogy, partial atrioventricular septal defect (partial AVSD), Transposition of the great arteries and congenital heart disease,\u2026<\/p>\n

 <\/p>\n

3.Subjects of application:<\/strong>
\na)Pediatric patients in poor, near-poor and extremely disadvantaged households must be operated and treated for cardiovascular diseases.
\nb)Having enough preliminary documents, including all of results of echocardiogram tests from reliable medical facilities.<\/p>\n

 <\/p>\n

4.Content and value of support for pediatric patient:<\/strong>
\n-Support 100% (free) of total cost of examination, surgery and treatment for pediatric patients;
\n-Support 100% (free) of accommodation cost for pediatric patients during the length of stay in hospital.
\n-Support 100% (free) of transportation cost during period of surgery and treatment.<\/p>\n

5.Number of applicants and duration of the program:<\/strong>
\na)Number of applicants: 52 cases
\nb)Program duration: from January 01st to December 31th, 2019.<\/p>\n

6.Preliminary profiles of pediatric patient required to be sent to \u201c1 week 1 heart\u201d includes:<\/strong>
\n(1) Application form;
\n(2) Certification of circumstance of pediatric patient issued by People\u2019s Committee of ward, commune:
\n(3) Copy of Household registration book (authenticated)
\n(4) Copy of birth certificate of pediatric patient (authenticated)
\n(5) Copy of medical record (results of test, ultrasound, radiography\u2026)
\n(6) Health insurance card (if any)<\/p>\n
